August 2 was Day 35 of my first round of NBE and just past two weeks into 500mg of PM. At that point hard erections through self-stimulation were nearly impossible to achieve. Hard erections were still happening in intimate two-party settings. Ejaculations were not difficult either way—even with a mostly engorged but still soft penis. Morning and nocturnal erections had basically disappeared. I loved that phase!
August 7 was my sixth day of 1000mg of PM. I wrote in my diary that I also noticed that my testes had shrunk some. Another great side effect.
Unfortunately, I had to take a break from my regimen for about a week and a half. By the time I was able to return to it, nocturnal erections returned. This is when I noticed that erections were now somewhat painful; especially when self-stimulating. The penis doesn't seem to want to stretch anymore. It's not as elastic as it was previously. Is this normal? Is this my "new normal?" Is this only the beginning of diminished elasticity and more discomfort when fully erect?
(On August 18, I was able resume my regimen with 1000mg of PM.)

Painful erections is a new one. I've never heard of it. Many people report testicular pain and shrinkage but not in the penis itself. Unless you masturbated for too long to achieve erections like when you weren't taking PM. In that case it's a purely mechanical thing.
Is that the new normal? No.
Erections can be back in full force when you stop PM (and other substances). What seems to be final is the shrinkage of the testicles but that doesn't seem to affect their ability to produce sperm. Stop PM for a month and your equipment will be as good as new.

I think I figured out what's going on. It's called "deep dorsal vein thrombosis" or "Penile Mondor's disease (PMD)."

"It usually presents as a painful cord or fibrotic area in the dorsal surface of penis accompanied by intense pain." "The etiology of Mondor disease is unknown but has been associated with many factors such as sexual activity, prolonged abstinence periods, direct trauma to the penis..." "Most cases resolve within 4 to 6 weeks..."
My pain isn't intense, but it could become that way if the size of the cord-like structure increases in size. My case was possibly triggered by my sudden lack of penile erections and activity.

I've heard of this happening with HRT. Basically, if you don't have erections regularly, things start to shrink and it can get painful.
Solution is to have more erections. You don't necessarily have to finish, but if you stop having morning erections, you need to get them some other way regularly.
