I would not suggest the birth control patches because they are not designed for MTF transitions. I think you're fooling yourself if you think herbs have less side effects. I switched to doctor supervised HRT because the last time I did herbs there were noticeable side effects.
I'm no doctor and like you I had to assess my health. The two drugs you would likely be on if you went doctor supervised HRT would be spironolactone (to reduce the affects of T) which is a drug commonly given to treat HBP and bio identical estradiol, which even take orally is prescribed to be taken under the tongue (clotting and liver issues are not seen with bio-identical estradiol).
I also decided that my health had to come first so I decided to loose weight and over 19 months I lost 100lbs. Then I started HRT.

For me HRT is more dangerous.
