Hi, try pumping to increase nipple size.  I've used a double lactation pump.  You have to get shields that are the right size - maybe quite narrow - Maymon have a good range.   You can use water based lube on the shields to help them stay on.  Daily use needed for gradual progress.  Many folks use supple-nips as an alternative.
