Sounds like breast buds to me! I had the same donut shape you described.

Have you thought about trying a topical estrogen/progesterone cream instead of PM? I started and my nipples and areolas got about 50% bigger within a month. Worth a try if you want to do something but don’t want a lot of tissue growth.

I've been using Estradiol cream on nipples daily for 7 weeks, still no change in nipples :/
