I know they are breasts. My shirts fit different, very little chest muscles.  They sag a little.  For the first time in my life I am self conscious with a tshirt on and without a shirt.

Sounds like breast buds to me! I had the same donut shape you described.

Have you thought about trying a topical estrogen/progesterone cream instead of PM? I started and my nipples and areolas got about 50% bigger within a month. Worth a try if you want to do something but don’t want a lot of tissue growth.

Where do you buy your estrogen cream?

Amazon. I’ve tried both the Smokey Mountain Estrogen cream and Bi-Est 5.0. I’m using Bi-Est now as I seem to get better results with Estradiol/Estriol mix rather than Estriol alone. My nipples are now almost 1/2 inch wide, and areolas continue to grow.

Hello! Do you place the both creams directly over the nipple or apply on the breast only? Or both? Haha. 1/2 inch wide, that's my current target.
