Usually a normal doctor regular doctor your primary care doctor whatever the case may be may have a sneaking suspicion you might be having difficulties identifying with your gender however no one will make a diagnosis unless they are a mental health therapist. 

Your primary care doctor might refer you to somebody in the mental health department that you'll have a consult with and that psychiatrist will be the person who ultimately says hey yes I think you are struggling with your gender identity and you're having these feelings of dysphoria or there is another something else completely going on. 

For me I use the veterans affair for my healthcare and when I told my primary care doctor that I would like to know more about trans and gender affirming care that the VA offers and that I was struggling with stress and anxiety and depression from not knowing or not feeling completely at home in my body she put in a consult with the va's mental health department who will ultimately make that decision on officially diagnosing me. 

However even if they don't diagnose me with gender dysphoria I can still receive gender affirming care because there are so many different things that fall under the gender umbrella. 

So I would say short answer your regular or primary care doctor might have a suspicion that you might be dealing with gender identity issues but they are not qualified to make that diagnosis you would have to go see somebody in mental health and have a few sessions with them for the therapist to make a diagnosis.
