I was much in your position about 8 months ago, and now I'm transitioning. I was on PM+SP for about 6 months before I got my letter from my psych and then shortly after got onto Spiro from the endo.
I saw my GP first before even starting PM+SP and mentioned I was transgender, so it wasn't much surprise when I wanted a referral for the psych and then the endo. You don't need to worry about doctors, they are professionals, there are some aholes and I've changed GP twice since then and found a great female doctor that is huge support.
PM can get you a long way in terms of breasts and slight fat redistribution, but you won't get all the feminizing features of proper HRT (most importantly for me, significant facial fat redistribution, that goes a huge way towards passing). And the PM route is much slower. I'm already noticing significant breast growh on HRT in 2 months! A long with some finer hairs and change in smell.
But it's a great way to start, see how you feel after 6 months, give yourself time. You will need a lot more time to come out to yourself and then others than you realise, it's a tough process. If you feel after that or longer that you need go all the way then proceed from there. Some people are perfectly happy on PM alone and find it helps them psychologically and they get to feminize.
