--------------------------------------------------------------------------------

People frequently ask whether feminizing hormone therapy, at the doses used in transgendered patients, fuels an increase in complications such as stroke occurrence, and whether smoking and/or marijuana usage influences this risk. The answer is that cigarette smoking does in fact increase stroke risk. The likelihood of a stroke is influenced by many factors including the presence of high blood pressure, arteriosclerosis (hardening of the arteries), and the presence of clots. Usually we think of stroke in terms of paralysis, such as that found in an elderly person or even a younger woman who has been using birth control pills for an extended period of time, where one suffers from paralysis of one side of the body. However, blood-vessel related disease in the brain can lead also to cognitive dysfunction and sometimes dementia of a type which is not Alzheimer’s disease, especially in the older patient. The long-term damage depends on whether there is blockage of the blood vessels (ischemia) or whether clots are sent to the brain (embolism).

The risk of embolic stroke is an important consideration when treating women experiencing menopause. When feminizing hormones are taken by mouth, even in the lower dose we often associate with menopausal therapy, there is a definite increase in the risk of embolic strokes. This was one of the findings from the Women’s Health Initiative study of menopause (WH I MS). However, this study was based on the use of oral Premarin and Provera. In contrast, when we look at the influence of non-oral estrogens such as estradiol pellets, patches, or gels used in proper dosages, there is no evidence of an increase in stroke, embolic or otherwise. Indeed, we have good reason to believe that non-oral estradiol therapy is a potent means of reducing the risk of heart disease and stroke. High blood pressure should also be treated when significantly present, but even then, non-oral estrogen therapy will often reduce hypertension.

This brings us to the question of whether estrogen therapy is more dangerous when the patient also smokes cigarettes or cannabis? The influence of cigarette smoking on estrogen therapy is profound. One pack of cigarettes per day doubles the rate of excretion of estrogen from the body. As a result the process of estrogen withdrawal is exaggerated with significant negative effects, such as increased inflammation. Additionally, we know that smokers who consume estrogens, particularly those taking combination oral contraceptives, are at significantly increased risk of excessive clotting, leading to deep venous thrombosis of the legs, pulmonary embolism of the lungs and even embolic stroke.

In this day and age we all have sufficient information available to us to know that cigarette smoking is incredibly hazardous to one’s health. Thus, the individual who is on high doses of estrogen while continuing to smoke is unfortunately either ignorant or self-destructive. With regard to the smoking of marijuana: despite much talk in the past about increased prolactin levels and diminished testosterone levels, the influence of marijuana very much depends on the purity and potency of the product, Regardless, marijuana is not tobacco and does not appear to carry the same risks and problems.

Knowing these facts, if one still chooses to smoke cigarettes, then that is their business. It is not for us to try to force them to quit, but they should for a wide variety of reasons. The individual who smokes cigarettes while simultaneously using oral estrogen is living life needlessly on the edge.

Hi Sammie, I'm back at last, nobody missed me then Sad
The reason for my absence was documented in one of your earlier threads and I am not going to update it in this thread but leave it for another time.

I am ashamed to admit this but I have been somewhat sneaky and deceitful to my wife that I did not disclose I was taking PM. I have been rumbled as she received a package whilst I was out assisting one of my sons with his garden. I received a phone call wanting to know what was in the parcel as it sounded like tablets rattling !
I had to come clean as she demanded to see them and then proceeded into WW3.
Now to the point of my post, what it didn't disclose for very good reasons was that I am taking 'Time Release' oestrogen & Anti-Androgen tablets and have been so for 3 weeks.
I chose this path to speed up the growing process and also for a certain level of convenience as they are consumed just before going to bed. As things stand I cannot with my hand on my heart say exactly how much improvement I have achieved growth wise. What I can report with certainty is that my breasts have filled out which is both visible and to the touch.
My wife insists the PM goes back or their will be serious repercussions, they have not gone back but neither have I started consuming them yet. I will have to start with them again as it's been more than a week of abstinence and I note a decline in breast pain with just the Time Release capsules. I obtained 3 months worth of the TR Capsules mainly to accommodate when on holiday in September but wanted to take them early to ensure I did not have any allergic reactions to them. None so far and don't expect any !

I have missed you all very much and I owe Clara, Annie and yourself a huge hug and a lifetimes gratitude for being so patIent and supportive.

Love to you all

Heather

Heather, honey, I had noticed and was going to send a Pm tonight to check on you, but you beat me to it! Lol
I am so glad to see you again, though I wish it was with better news.
Still, time release E is good. I wasn't aware it existed. It sounds like a great idea. Still, it does continue in any tablet form to process through the liver. The new procedure of course sends it directl into the bloodstream which should make it so much more effective.
But the real news is that you are on E now!
Yay for you, sweet friend! You will have to keep us posted on progress as well of course, as to how things go with your wife.
I shall hope for nothing but the best on both fronts.
Mostly, though, I am just so glad to know you are safe and sound.
Hugs
Sammie

Thankyou Clara , Eva & Sammie, yes I am home and with my beloved sisters who I admire so very much.

I overlooked the Time Release you were referring to bypassed the Liver unlike the ones I am taking. That may explain the reduced pain behind nipples, just a hunch.

I will keep you informed as to my progress.

Clara, I owe you big time and will send you a PM dear sis.

I am not going to hijack Sammie's thread but what I can say is that my wife refuses to acknowledge my GID and thinks I am moving the CD'ing goal posts. She flatly will not travel on the Transition road and not accompany me on any specialist appointments. As all you girls have said, " it can be a lonely journey" and from this angle that picture is showing clarity.

Love you all

Oh, Heather, I do feel so much for you, particularly when my own situation seems to have clarified so wonderfully. My own DW says unambiguously that her alternatives are to leave me, which she will not contemplate, or to support me fully as we travel along this road in partnership. Your wife may see in the end that her alternatives are similarly stark. Her refusal to believe in your GID seems to be an emotional rearguard action against a problem she can't presently cope with, otherwise she would surely be at least arguing the issue or offering compromises rather than just giving orders. Being of devious mind, I'l see if I can think up any tactic that might be helpful, but it does seem to me that the present situation must evolve with time and your best course may be to continue to stand back a little as you have done and let time and providence do their work. I do so wish I could offer you some real comfort.

Thank you so much Annie, you along with Clara & Sammie have a gift of offering warmth an comfort in your advice that goes straight to the heart.
I promised Sammie I wasn't going to hijack her thread and I am in danger of doing just that by blurting on like this.
I will create a thread of my own for comment, please please please don't look upon me as all 'gloom & doom' it's just a bad phase I am going through. If I can end this post with a song I heard this morning titled " why won't you let him be what he wants to be" sung by Jodie Miller. It's worth a listen and applies to our own desires and expression. I am going to record it and keep playing it on my music system as a subliminal message to my wife! Big GrinBig GrinBig Grin

Thank you dear friend and Sister

Love
Heather
