I suggest you switch to some other T blocker at some point, there are better stuff than spiro out there. Cypro or Bica might be better. Take estrofem buccal/suglingual, it absorbs nicely like that and it works fast, so lot of ups and downs with it. Obviously I advocate for injections as the best option, but sublingual pills or gel are far easier. Patches are crap and overly expensive.
With BO start low and slowly ramp up your dose. It can work as low as 500mg, but optimal is said to be 1500mg per day. I'm on 2300mg right now and it has felt really good, maybe not necessary to go that high. 1000mg and above is where it really kicks in well.
About your possible HRT, don't do the usual tiny bit of estrogen and boatload of Spiro kinda thing, its not really good. Better to get more effective T blocker which can be taken on tiny doses and if you're on pills, you might want to work up to 6mg per day. Higher likely isn't needed or even good as pills always have quite a bit of liver metabolism involved. If you can, switch to transdermal or injection at some point, those methods are simply better for almost everybody. At some point you might want to consider progesterone.
About NBE, glandulars work fine along with HRT. Amino acids which boost growth hormone and growth factors are defnitely a thing to look into. Also the MSM + vitamin D3 + calcium citrate combo is known to be very helpful. From herbal stuffs you might want to look into prolactin boosting stuffs, don't overlook prolactins role in breast development.
