This seems to be a common question asked by bio-males new to this site, so here are some comments from my experience, since I have likely pushed this whole process about as far now as anyone.
The short answer is yes... eventually...probably.
Keep in mind that herbal NBE is a form of the same exact process involved in synthetic hormone HRT (Hormone Replacement Therapy), or as it is commonly referred to in the TG world, "transitioning."
To be clear, "transitioning is a slow process, and is not the same thing as a "sex change", although the two are related. Normally the one (transitioning) leads to the other (Gender Reassignment Surgery or GRS). But that is not always the case, especially today. Some bio males opt to remain in one stage or another of the HRT transitioning stage forever and never proceed to GRT. Some prefer to keep their "boi bits" while developing other aspects of femininity.
Essentially, males grow breasts and feminize their bio male bodies by reducing Testosterone and increasing Estrogen.
That's it.
Taken far enough, this process will lead to what is called Hypogonadism. (http://en.wikipedia.org/wiki/Hypogonadism)
When this occurs in a male, the testicles atrophy over time, and stop performing. Infertility will eventually occur. Even the penis may shrivel somewhat and diminish in size.
This is what occurs through synthetic HRT.
When we begin on herbal breast enhancement (HBE), we are basically doing the same thing, though through the use of over the counter herbs rather than prescription required synthetic chemicals. The herbs are weaker than the synthetic drugs, but the process is the same. We are reducing T and increasing E. It is the same thing, only it takes longer, and requires more patience and know how, and it is completely private and within our control since no doctors or therapists are needed.
But just the same, without precautions, impotence and infertility will eventually most likely result.
Of course many factors play a part in all of this. Age is a big factor. As males grow older, the body normally stops producing as much T and compensates by increasing E, all quite naturally. This is why viagra is so popular among males of a certain age. In some ways older males start this process ahead of the game.
On the other hand, older males also observe a decline in natural production of Human Growth Hormone (HGH). Since HGH is also required for breast growth, older males face a setback in this department.
Breasts normally grow the most in pubescent bio females. HRT is an attempt to simulate a new feminine puberty in bio males. The older the male, normally the harder it is for this to occur.
Still, it can and does occur in many.
Now, to describe my case.
I began on PM at the age of 57. Through the next 3 1/2 years I progressed through various amounts reaching a plateau of 3000 to 4000 mgs, daily, along with approximately 500-600 mgs of Saw Palmetto. Ostensibly, the PM acted to increase my E, while the SP acted to reduce my T.
So, what did this do to my male function?
At first, nothing,
For at least a year or more there was no difference.
In the second year at some point I began to observe a change in my emission. The liquid changed over time from thick and creamy, slowly, to a thin clear fluid.
During this time it became more difficult, though not impossible, to attain erections, and my desire began to wane.
During the next year the amount of emission began to decline.
By somewhere in year three, the amount had declined to very little. Eventually, there was no emission at all. Among some TG girls this is referred to as "shooting blanks".
As this occurred, my desire for sexual release declined to virtually non existent.
Today, nearly four years into this process, my testosterone measures at roughly the same a bio woman, well below the normal male range even for a man of my age.
A recent series of tests by an endocrinologist confirmed these findings.
Since I already have children, and since I had enjoyed my male bits to an extreme in the previous years (not meaning to brag... well, maybe a little...tee hee), and since it is my desire to pursue feminization as far as possible short of GRS, I am quite comfortable with this. However, for the sake of curiosity, when consulting with my endocrinologist, I asked if this condition (erectile dysfunction) was irreversibly permanent. I was told "no" that is was reversible through testosterone supplements, should I so desire. I did not ask about the state of my fertility, so that question remains unanswered in my mind. It is of no consequence to me since with two almost grown offspring (17 and 21) I consider myself well beyond wanting anything else to do with producing offspring. But, obviously, this may well be of consequence to younger bio males still potentially interested in one day fathering children. I would advise such bio males to take the precaution of freezing some sperm either prior to beginning NBE or early in the process.
Now is this the only possible outcome?
Of course not.
Many members, while on a program of NBE, attempt to forestall impotence by taking regular short breaks to allow their T to rebuild. This seems to work for many. Others, perhaps mostly younger, but certainly those with a higher natural T count, may take much longer than I did to reach this point.
Lower dosage amounts of herbal E will also lengthen the time frame, perhaps indefinitely.
But do understand. Taken far enough, the results are almost certainly going to be the same.
Just as all new members are encouraged to think long and hard about how they will actually deal with the reality of large breasts should they successfully grow them, new members contemplating this program should also be well aware that they are following what is by nature the very same path as those on synthetic HRT, and that impotence and loss of male function is a very likely eventual result.
Of course... I am no a doctor (though I have played several on television...tee hee...really). Your results may vary.
